While there are few definite details in the vision, Steve sees himself still wearing the Captain America uniform and wielding his shield. He does not pass away quietly in bed at home, and going down fighting seems to be his destiny. He already knows it’s a battle he can’t win, yet in true Steve Rogers fashion…it does not prevent him from taking the stand, all the same. He sees himself telling others to “run from this place as fast as they can” as he acts as a final defender, likely still acting under orders from the avatar of Life itself.v
To add further credence to the vision, one of Cap’s old injuries returns moments before it happens in the current timeline. As he raises his crystal shield to view Life’s true form, Death attacks his other arm, resulting in total loss of movement and feeling. Although Lyra heals it later, she warns him that it will return, and that is the precise injury he sees in the vision. From this, it’s clear that Steve’s death really is approaching.
